Complete the note after the activity
Use the blank worksheet below. Enter the task that actually occurred, not the activity you had hoped to finish. Leave a field blank or mark it unknown when you did not observe it.
- Name the skill or activity without assigning a diagnosis.
- Describe the materials and how the learner responded.
- Record adult modeling, prompts and other assistance.
- Reference a dated sample without putting sensitive records in a public form.
Separate a result from an interpretation
“Answered after an adult model” and “answered without assistance” describe different conditions. The illustrative entry below demonstrates that distinction; it is not an actual child’s record. Do not add a percentage unless you recorded both the correct responses and the total opportunities.
Review the conditions before comparing notes
Check whether the task, assistance, response format and materials changed. If they did, record the differences. This worksheet does not determine whether an intervention caused a change or whether a child has met an educational goal.
Keep formal requirements separate
IDEA specifies measurement and periodic reporting within an IEP. This optional homeschool note is not an IEP progress report or a statement of your state’s homeschool requirements. Verify any reporting obligation with the responsible school, state or program before relying on the format.
Bring a precise question to the next conversation
Use the record to show the instructor what task was attempted and which support was provided. Ask how the next activity or measurement should be chosen. If you need recurring organization, inspect HM’s current membership description to see whether its tools match that need.
Homeschool progress note
An optional record of an actual learning activity. Use descriptive observations; this worksheet does not diagnose a difficulty or establish mastery.

Official checks before you act
Because homeschool law, special education services, and funding rules vary, verify requirements with your state education department, local district, official ESA or scholarship program, and qualified providers. Keep screenshots or PDFs of official guidance with your records.
