Use school terms only within their scope
IDEA requires an IEP to describe progress measurement, special education, related services, supplementary aids and services, and individually appropriate assessment accommodations. Those requirements apply to an IEP; they do not make this optional homeschool log an IEP, official accommodation approval, or required state record.
Original planning tool: accommodation log fields
Use one row per task or activity. The log documents what occurred; it does not determine why a support was needed or prove that the support caused the response.
| Field | Example format |
|---|---|
| Date and task | 2026-09-10 — explain the water cycle |
| Learning target | Sequence and explain the stages |
| Access support | Directions read aloud; diagram available |
| Response format | Oral explanation while pointing to diagram |
| Assistance | One direction repeated; no answer supplied |
| Observable response | Named three stages; reversed two in sequence |
| Next question | Ask instructor how to practice sequencing separately |
Label the example accurately
The water-cycle entry is hypothetical and only demonstrates the fields. It is not a real child record, research result, or recommended accommodation for every learner. Replace it with facts from the actual task.
Compare only reasonably similar activities
Before comparing entries, check the target, directions, materials, response format, difficulty, time, and assistance. If conditions changed, record the difference rather than attributing the result to one support.
Keep the completed log private
Use initials or a nickname on a working copy and store the key to that name separately. Do not place diagnoses, full birth dates, school identifiers, clinical notes, or provider records in a public form. Share a relevant excerpt only through the agreed process.

Sources for this page
The IDEA sources describe IEP content and review of existing data in public evaluation contexts; they do not mandate this homeschool log or validate its fields. The planning tool is original editorial material, not a validated assessment or official form. This revision remains pending educator review; verify individual recommendations and current state, district, program, and provider requirements before acting.

Official checks before you act
Because homeschool law, special education services, and funding rules vary, verify requirements with your state education department, local district, official ESA or scholarship program, and qualified providers. Keep screenshots or PDFs of official guidance with your records.
Frequently asked questions
Is an accommodation log required for homeschool?
This page does not establish a requirement. Check the current state, school, testing, and funding-program rules that apply to the actual activity.
Does one successful entry prove the accommodation works?
No. It records one activity and its conditions. Ask the responsible educator how to review repeated evidence and make an individual decision.
Does SEH collect this log?
No submission or storage feature is provided by this page. Keep completed records in the family’s private system.
